Free Medical Excuse Forms
Right off the bat, lets rid ourselves of all moral guilt for finding a fake medical excuse note on the internet. Let us assume that you really were sick but doctors cost money and you just had a cold. In order to prove you were sick for your job or school, you need a doctor's note.
This can be complicated depending on who requires the note as some places of business ask that you bring in the invoice for the doctor's appointment along with the note. That could be tricky and we are not going to cover that here.
Fake doctor's notes are hard to find on the internet and frankly you are better off making it yourself. This allows you to customize the note as you see fit and does not require anything more than a word processor like Microsoft Word and a printer.
First, you need to figure out the size of the note. Some offices offer full page notes while others have notes about the size of an index card. It is easier to make a full page note but feel free to make a smaller one if that is what you are used to.
Then, enter a medical excuse heading on top of the page. This could be "Medical Excuse to Return To Work" or something like that. This is usually in a larger, bolder font at the very top.
Next, enter the doctor's name or the name of the office or hospital, the address and telephone number. If your place of business typically calls your doctor when you have been sick, it might be a good idea to put the phone number of a friend instead of your actual doctor to avoid being caught.
Then, enter the "illness" that you were out with and the dates that you are excused from work. Print it out and you should be good to go. I should point out, however, that you are better off actually going to the doctor or clinic and getting a real doctors note and real medical advice.
